24 Hour Emergency Assistance

Our team of trained professionals is available 24/7 to respond with a sense of urgency to any emergency that may occur in the home or community potentially impacting our clients. Our goal is to provide our clients with the support and resources they need to navigate emergency situations with ease and peace of mind.

What is included in our 24 Hour Emergency Services?

  • Rapid response to emergency situations
  • Coordination of emergency medical services
  • Assistance with evacuation and relocation
  • Support for clients and their families during crisis
  • Follow-up services to ensure the safety and well-being of our clients
  • On-call counseling, on-call problem-solving, and immediate response for assistance at a person’s home due to health or personal emergencies that are not related to an injury or an immediate danger.

*Please note that this service is not a replacement for 911 and should not be used in place of calling emergency services when necessary of immediate danger.

How our Basic Services Process Works

  1. Call us to schedule a brief phone consultation to discuss challenges and needs.

  2. We will verify insurance providers to ensure service is covered.

  3. We will schedule an intake appointment to capture information that will be used to develop a person centered housing plan. 

  4. We will develop a working plan to understand needs and build a person-centered plan that is just right for you.

  5. We will work with you on reaching each goal within the Individual Service Plan and provide resources that can help you obtain each goal.

  6. We will work continue to support individual needs and re-evaluate and continue to develop additional goals that can support overall well-being.
